In worse cases they have extra of something and they'll then pay to have them altered just enough to fit the theme and ship that. Other times they have contracts with vendors and they specialize in particular things that just don't fit the theme of the CE. If procurement denies or cannot find a vendor to make what they want for the cost they'll scrap it and go with something random. In some cases marketing doesn't even get what they want. The choices are solely made based off cost breakdown and availability. This is how you have choices being made by people who haven't even read the books.

They're told they need to make a collector's edition and they have say $70 worth of budget to spend on gear to put in the collectors box that'll sell for $300.
